Every puppy sold by Pet Pros is assigned a unique 5 digit ID number called a "Store ID". Your puppy's Store ID number can be found in the upper right corner of your Guarantee/Purchase Contract, or on the outside of your puppy's folder.
Registration papers records are only available for 5 years.
Do not call one of the stores about registration papers! The retail stores do not have any information about registration papers and they will not be able to help you if you call them. All issues with registration papers are handled by the corporate office via this web site only. If for some reason this form does not work, you may also send an e-mail to Papers@PetProsLV.com, or a fax to 702-926-9615. Please be sure to include your puppy's Store ID number in the e-mail or fax.
